Question Send 28-75 back to Tamron to recalibrate. Process?

I think my 28-75 needs some lovin' from Tamron. For those that have sent theirs back, whats the rundown? What will I need before I contact them: receipt, warranty card, etc.? Cost?

TIA
Sherwin
