Quick and easy, though I have to say that it would be helpful if the scale went to f/11 or f/16 (since that is the range that I shoot in when stacking teleconverters). Still, the math isn't hard. Close down the aperture a stop, slow the shutter a stop.

The moon can be exposed with the Sunny 11 rule of thumb...1/ISO at f/11 (the Sunny 16 rule underexposes the moon)
I don't think the moon's phase has anything to do with exposure. The lit side of the moon is always the same brightness, it is only the percentage that is exposed to the sun which varies and that does not alter the exposure of the lit side!

The moon's height in the atmosphere has some effect on exposure as well. In general, the lower in the sky (closer to the horizon), the less bright the moon is. It's not a huge difference, but it needs to be accounted for.

You do need different exposures for different phases of the moon. I agree with vondo.
