Default Focus issues: User error or Lens?

I bought a 17-55 about two months ago and have taken out now three times to shoot. I did all my shoots with my 50mm 1.4 prior to getting this lens. I am having focus issues BAD with this lens. Im not sure if this is user error or lens error. Ive never had this problem before. Also two times now the auto focus just quits working. I literally have to turn off the camera, take off the lens and remount it to get the focus to work again?

Anyway here are two sample images from my shoot yesterday and then sample images from my focus test I did. Note with the two sample images that I ALWAYS use the corner of the eye to place my focal point using one of my corner focal points. And you can clearly see where the focus lands and this happens image after image. Test charts are on a tripod with a timer to make sure nothing changes during the shot.

Not sure what you're seeing, but I'm not seeing anything wrong with the two pics of the girl.
I don't pay any attention to focus chart photos.
