What info to ask on client info form?

I am new to the business of photography and am in the process of developing a client infomation form. Other than name, address, phone #, email address, what is on your info form? Any advise will be greatly appreciated.

birthdays could be useful in future marketing strategies, but most people dont bother to fill that section out.

For future reference:
Supervisor's name, address, phone #, email.
Did they refer me to someone? Results of the referral? What Association? Associates name, address, phone #, email.
What did they use the images for?
What was their budget?
What did I charge? How soon did they pay?
Notes on the shoot.

Great list if your doing commercial photography. since the OP was new to photography I simply assumed they are doing regular portriats.
