The reason I'm asking is that I love it and want to shamelessly rip it off ;] Your deconstruction is pretty much on the money. There's a shoot through umbrella camera left, almost touching my face and slightly forward of me. Camera right has an unmodified flash coming from fractionally behind. No contribution from ambient. I'll attach what it looks like pre the heavy tweaking in Lightroom.
