Lowest-profile FastenR-style attachment?

A friend gave me a BlackRapid-style sling strap that I'm in love with. However, it came with a mounting plate that's large and makes it a little awkward to store and take pictures in portrait with a grip.

I'm looking for the lowest-profile FastenR-style attachment. It can be DIY or knockoff or whatnot, I'm not concerned.

That looks entirely decent. Rubber washer, locking washer, screw shouldn't be too hard to find. What is the D-ring/clip thing called?
