400 F/5.6, it fits best that way. If I'm shooting with shorter glass I normally don't bother with a bag.

18-55(kit lens) mounted because I don't want to mount the ef 75-300 f/4 - 5.6 III all the time. Caps always on, on every camera.

Umm. the only one I own.. the 28-135. lol It will probably still be that one that stays on my camera for the most part when I get my next lens. (100-400mmL)

The only time I carry my camera in a bag is when I'm traveling. If I'm out shooting, the camera is always on the strap with a lens mounted, even if I'm not actively engaged in taking pictures at the moment.

Under normal circumstances, I carry a pair of 40Ds with a Tamron 28-75 on one and a Canon 70-200 2.8 IS on the other. A third lens in my bag is the Tamron 14mm 2.8 and the Canon 1.4X and 2X converters. Sometimes I will have the Canon 100-400, either as a substitute for the 70-200 or in addition to it.

which lens - whatever was on it after the last time the camera was used.
why - because I couldn't be bothered changing lenses
lens cap on or off - whatever I could be bothered doing.

Besides, you can't predict the next lens you might need and chances are it will be too long/short, so it doesn't matter anyway.

Yep, me too.
