If it's supposed to be there- I don't clone it out. Pimples I will (if they're really noticeable). Moles, freckles, scars... those stay.

Temporary marks are removed, permanent marks (freckles, moles, lines) are left unless removal is requested.

Yes,if requested/assignment pending. You are actually reminding them more of their imperfections when you clone freckles,moles, and lines etc.... out

Blemishes - I always clear up...
Scars - I will (again) if it's brought up ;)

Definitely not freckles for me. Not because it's labor intensive, but because that's what makes that individual different.

If it's some mole, zit, scar, I'll ask the bride what their preferences are prior to correcting them.

I wish Portraiture could work without totally ruining freckles but with them I always end up retouching manually in Photoshop.

Freckles I wouldn't cover up, for the rest, blemishes, moles, ect I use one general measure to decide, did they try to cover it up with makeup? That tells me if I should take it out.

Otherwise I'll try to minimize the appearance of scars or moles. I don't want to change the way they look, but most people are going to remember a brides huge smile and how beautiful she looks in her dress, and that's what I try to make the pictures reflect.

Of the few weddings I did I did not. However the groom from one wedding had some rough skin on one side of his face. It looked like he had acne at one time his life. It was a tad distracting with the close ups so I lightly cleaned it up a bit. I also agree that freckles are not imperfections. Unless the couple specify it I would just lightly clean up scars, a pimple that just happened to show up that day, etc just as the other posters suggested.

Freckles aren't imperfections in a politically correct "everyone is beautiful" sense, but medically they are.
