Does it fire when its off the cold shoe?
Make sure none of the Speedlights contacts are shorting out on the cold shoe. This will damage your Speedlight and also could explain why its not working correctly. If any of the pins are touching the cold shoe you can prevent shorting but sticking a piece of electrical tape over the part thats near the pins on the Speedlight.

Black electrical tape is probably too thick. If you use 3M Magic Mending tape, it is thin mylar, which is an excellent electrical insulator.
