When using an 580 EX or EX II on a Canon 5D II, what is the default lighting ratio? For example, when in ETTL mode, does the flash automatically match ambient or go a stop under ambient?

It depends on the exposure setting. If you are in Av, the default flash effect is to match the foreground flashed subject to the background ambient exposure. You can alter the flash effect using flash exposure compensation (FEC), and you can independently alter the ambient exposure using Exposure Compensation.

When I use the flash indoors I usually use it in M mode. I pick the f/stop based on the depth of field I need. I adjust the ISO and shutter speed to bring up the background light. You can use FEC to adjust the flash exposure on the subject.

I never use flash in Av or Tv mode indoors. In those modes it seems to try use the flash only for fill lighting. It sets the aperture or shutter speed based on ambient light and then tries to fire the flash to match. This is not what I ever want indoors.

I think the flash will also work in P or A modes indoors, although I don't have much experience with that.

I completely agree. I exclusively use M indoors with ETTL flash. This lets me set aperture & shutter speed to get the amount of ambient exposure I want or eliminate ambient entirely. Outdoors I usually use Av, and the flash then provides fill. Exposure compensation and flash exposure compensation let me adjust ambient versus flash in any way I wish.
