I think having another cable defeats the original standards of the manufacturer. Have any of these straps failed?

Yes, straps fail, Black Rapid or otherwise. It is really the loops that fail, the screw lock fails, you don't tighten down the lug on the tripod mount, etc. Having a 2nd unobtrusive method of linking the strap itself to the neck strap lug on the camera provides a very stable safeguard without adding any bulk or issues with sliding the camera up/down the strap.
