But when the speedlite is mounted off-camera the shoe isn't hot.

And yo be just complete the trigger is fitted to a coldshoe or screwed on the mountingpoint.

And to be utterly complete, there are now hotshoe flashes with build in trigger and that are mounted directly to a coldshoe or screwed to a mountingpoint.

I removed the hotshoe on my Godox AD360 so I can't use pictures lit by it in this thread. By replacing the screw-in socket with the hotflash again. By doing so it qualifies again for this thread. ;-)a

I don't think your light is harsh. But it's not flattering because the light is on axis with the lens move it away from the camera.

The light is harsh. Even off axis it would be harsh.

But harsh light off-axis can work. :)

I had the flash on a bracket connected with an ETTL cord. I think bounced on the ceiling gives better results.

You need a bigger light source, and you need to get it off the camera and on a stand. A bracket and bouncing are fine for event work and weddings but they don't provide the kimd of light you need for the subjects you often post.
