There's no creep, lens doesn't extend.

All the action happens inside the lens, so there's no tube actually moving in and out of the body. No lens creep. I'm not sure it's even possible on this kind of design, but I can't say for sure. Mine is at least 6 years old now and it's solid as can be.

I looked this up. So it attaches to the front of the lens like a filter would via a stepdown ring? Does the 70-200 then function as normal, only focus close because of the microscopic lens? How stable is the image or does it wobble like crazy?

Yes, microscope objective is mounted via a step down ring.
My insects are dead (so they can't moving), camera is on automated macro rail (check my DIY Album https://www.flickr.com …/albums/7215762​7644689149 (external link)) - not handheld and my shots are stacking images.
