Yes, but not all of them. There are a number of State operated full service stops with toilet facilities, some offering features that extend all the way to restaurant plazas and gas stations, but that scope varies by State and applies only to very major thoroughfares and pay roads. Some of the older routes, or less utilized roads, often have rest areas and you depend on gas stations for the toilet. There are even dedicated For-Profit Fuel/Food/Toilet stops. Here in Texas we have a chain that built an empire on providing clean toilets and - no joke - they are fantastic. As a talking estimate, there's a full service stop after crossing every State Border (on major highways) and then every 50-80 miles (that's like every 300,000 kilometers) within the State.

The States run a solid operation in maintaining safety at the stops. I'm sure theft crime occurs but when I was a punk in my younger days, I've been chased out by the Staff after a night of drinking and once was even woke up for a search after the dogs indicated my car for drugs. That's not my thing so, #innocent.

I try to use the smaller picnic areas along the drive to stop and make a coffee, or a dehydrated lunch. They're usually empty, usually very well maintained, and, I think, often rather scenic. At that rest stop outside Sanderson, I saw maybe one car every 5-10 minutes. (aside: The lead character in No Country for Old Men was from Sanderson, TX)

You got me spooled up, so two other interesting things about car travel in the States. First, is that if you have a certain kind of camper, you can stop and spend the night in a Walmart parking lot. I don't know if that's official Walmart rules, but it's done and fine. Second thing is that any drive is packed full of Historical Markers. There are little plaques and tiny dirt pull-off areas that highlight something unique to the area. There might be an empty field where the home of the County's original founding family lived, so there's a plaque. Might have been a local rebellion at some point, so there's a plaque. I haven't had the patience to stop at them, but interesting to do some time.

Here is another road near the one I posted a picture of a couple of days ago. This one goes up Mount Mensura, the highest peak in that area. For some unknown reason, in the three trips I made there I never saw anybody driving on it.
