I also have the latest firmware, 1.2, but I don't think your issues are related to the firmware. One thing I noticed with the R5 is that it seems to require a higher shutter speed when using the Eye-AF. With my 5D IV, I'd be able to get well focused shots like the above with the shutter speed of 1/800 comfortably, using the spot AF point. With the same setting in R5 in Eye-AF, I suddenly noticed a lot of OOF shots, so now I'm shooting the R5 at 1/1250sec. I haven't had a chance to shoot much lately since the discovery, so I'd need to confirm my own findings with further tests.

Well with PEOPLE EYE autofocus, l've been shooting portraits for months now at 1/250th. Obviously not with this lens so that is why I felt the issue was specific to this Sigma lens. Very odd and frustrating. I've missed so many shots. I may take this over to the R5 thread and ask there.

When you post the issue in the R5 thread, be sure to include what your R5 settings were, i.e., what SS, AF, etc. Also check to make sure that the OS was on in your Sigma lens.

OS was not turned on, the lens / camera was tripod mounted and I was remote shooting with a trigger. Do you think OS should be ON while on a tripod?

This a different bird on the same branch as the the out of focus bird (Red-breasted Nuthatch), what did you do differently to get this bird in focus?

Yes, nearly the same spot, different day. Removing lens from the EF-R adapter and then removing the adapter itself from the camera then reattaching both solved the issues. It was definitely an issue with the adapter as I experienced the same issue with a different lens. Once I re-attached everything it was all good.
