On the form you will notice the choice to add the 5D and 24-105L kit, then you'll just use the single barcode from the box.

The lens and it just has a photocopy of the 5D box inside it in order to claim the cash back... mmm, I wonder if Canon would give me the cash back for the 5D too (still sitting in the shop)?
