Mind sharing how you focused, that seems to be my biggest problem, a hit and miss event. Here are some focusing hints that work for my 7D and 400/5.6L lens when using both of my extenders:
While LV AF can get you very close to perfect focus, it rarely is perfect. Once AF gets you in the right neighborhood, you can tweak it with manual focusing. This is very touchy and you will need to wait for the vibrations to ring down before judging how close the focus is to perfect. Sometimes I spend a lot of time tweaking and other times, I hit it right away. One of these days I plan to try AF with a laptop computer running EOS Utility. Just as important as razor sharp focusing -- is having a rock solid tripod and head -- otherwise, vibration will eat your lunch. While the following sounds like overkill, it really isn't.
