Here's a quick one I just did to see if I could. Took about 20 minutes.
Clipped the animated section I wanted out of the video in premiere. Exported to AVI.
Imported AVI to virtualdub.
Export from virtualdub as GIF.
Open GIF in photoshop as well as my still photo. Copy and paste still photo into GIF on it's own layer, resize/position to line up eyes.
Selected eyes with lasso and feathered selection. Delete selection.
Save for web.
Obviously my head wasn't perfectly still and it is noticeable but this could be corrected by lining up each frame of the GIF.
