The subject in question has kids, and I think that what's being seen here, is a combination of things. A) light fall off, B) the model has a rather large rack, and the tops of her breasts are tanned oddly compared to the rest of her and C) the fall off with her stretch marks, well that works against the image. As far as the "fall off", not really sure, perhaps the reflector was positioned poorly. The shot was taken at "synch speed" of 1/200" with a 5D mk II.
