Ok, first off, I am at work so I can't check what my husband is telling me  but I know I tried this yesterday & had the same problem. We have two Nikon SB24s on stands with PW IIs synced with cables, then we have my 580 EXII on camera with another PW II connected to my camera (5D MKII) with the sync cable. We can NOT get the flashes to fire with my 580EXII on camera (except for the 580EX II), but if I take it off & attach the PW II to the hotshoe, then all the flashes will fire. We tried having the 580EXII in both M and ETTL. Ummm, can't think what else now I need to add as far as settings but can check when I get home. Any ideas? Thanks!

Are you plugging in to the "camera/flash" plug on the PW?

Assuming you're connecting to the PC socket of the camera...

The setup should work, but those PC connections are notoriously unreliable. Try gently squeezing the sides of the PC connector together with a pair of pliars and reconnect it.

I highly recommend a screwlock PC connector such as this:

This always happens to me: I post a "problem" then figure out--I'm just an idiot. :) We had the sync cable going from the PW to the FLASH not to the CAMERA. Once I connected it to the camera, everything works! Sorry for the senseless babble!

At least the PW wasn't in the hot shoe backwards like the last guy.

Well, once, long long ago (ok, not so long ago) I couldn't figure out why my on camera flash wasn't firing, and I had it on the camera backwards!

I live in a glass house.

Who would honestly think the controls are supposed to be facing you so you could get to them rather than at your subject who probably thinks they're cool looking?
