Control points, if you don't know how to use them then learn. They great feature on all of the Nik plug-ins!

Also Nik's site has some video tutorials on their software plug-ins (if memory severes rignt the define tutorial goes into using control points to selectively de-noise

You can also apply the sharpening to the whole image and paint in the effect using the standard layer mask back in Photoshop.

The Nik Control Points are a fantastic time saver! They are absolutely the fastest and easiest method I've seen to create and adjust layer masks and applying a vast array of features.

Sure you could do much of the same in PS alone, but why waste the time? You could be spend more effectively doing plenty of other things.

The Mask View in the Nik products, shows you precisely what the effect of the Control Point(s) will be.

Also launching the Nik plugin from PS as a Smart Object, preserves every step of the entire NIK plugin editing process. This is really handy for subsequently going back in and making changes.
