It looks like he held it in between the fireball and the subject and shot the light through it. It basically acts like a 1m diameter softbox when you do that. Nice light, but you can't really tell that the fireball is behind it. The hot hands were from the direct light from the fireball that got around the diffuser.

Because if light shoots past the diffuser it is a lot lot hotter. I just metered the light at his head and not hands level. As they were closer to the diffuser I guess the exposure was higher than at his head level. So they appear hot in the image.

Dome can be removed with no screwdriver, but it leaves the dangling spring clips on the flash head.
