Best tips to get in a flow/rhythm for portrait shoots?

I'm looking for the very best tips on how you guys achieve a smooth flow when shooting portraits?

Once tip I learned today is not to review the LCD so often. Just keep shooting like 10 different images, then review. This allows the subject to get into a groove and relax a bit, rather than interrupting the flow.

Have a plan put together before getting started. Nothing interrupts the flow worse than standing there trying to think of what to do next. As far as looking at the LCD, I'll only check it occasionally just to verify exposure.....it is nothing more than a quick glance and the subject probably doesn't even realize it (I have my LCD review turned on so every picture with histogram briefly displays). I wouldn't try to examine an actual images on the LCD during a shoot.....that's just kinda rude IMO :)
