How do you get that super creamy look while maintaing a super tact sharp photo?

I'm not just talking about creamy skin but an overall creaminess to the whole photo, like not an ounce of noise and a "like you're right there" look. Oh boy this doesn't make too much sense does it lol?

What you should do is use an edge mask so that only the edges are sharpened, while the smooth gradations in the skin are not. I do this on almost all my photos in Photoshop CS5. This gives you really sharp eyebrows, eyes, mouth, and teeth, but very smooth skin. Here is one web site describing the technique:

Also, there is a really good thread here in POTN called "Really Smart Sharpening" that gives an in depth explanation of the technique. You can even take this a step further by copying and inverting the edge mask and using it to mask a subtle Gaussian Blur that avoids the sharpened edges.

I don't really understand the question.

Do you mean why the background is OOF? Or the skin?

That look can be easily achevied with fast lens and wide aperture (if this is what you are asking about)
