Quick questions about magazine advertisement terms

I just want to clarify a few things, any help is appreciated.

1. Does "Insertion" mean the number of times the ad will be placed?
2. Photoquote gives a pricing quote for EACH insertion right?
3. Do you really need to know the actual magazine(s) where the ad will be placed or just knowing the total circulation is enough?
