It was for all 5D. Mine was in the last batch manufactured and as soon as I registered it Canon asked me to send it in.
I'm not so sure most people ignored it. Many just waited for it to break before getting the fix done.
A lot depends on where the user lived, the mirror fault only occurred if the camera was used for an extended period in a hot and humid climate, most owners not living in such climates probably didn't bother. Those living in areas subject to those conditions would probably take up the fix sooner rather than later though.
Here in the UK (known primarily for being cold and wet, as opposed to hot and humid) there was no recall, and Canon didn't officially offer a free fix unless it broke. That was simply because there was very little chance of a UK based owner having a problem and having to "fix" tens of thousands of cameras, that were never going to fail, would be a pointless (and expensive) exercise. Unofficially, if you contacted Canon UK and said that you were going overseas into such an environment, they would provide the mirror upgrade free as a preventative measure.
My 5D is one of the first ones, has well over 100,000 clicks on it and has never had the mirror done, nor am I concerned about it needing to be done.
