Oh wow, the same thing happened to me yesterday, I didn't know what was going, tried another lens and it worked fine so I put it back on and it started focusing again, I will be getting the 5D3 tommorrow so I can update the firmware then.

I had the same issue until I flashed firmware; no problem since and the flash was painless.

I think this pancake is going to be my primary lens @ PAX in a few weeks mostly due to weight; looking forward to it :)

with 6d, t4i, or 5dmkIII; you download a firmware update for the 40 f/2.8 to the root of your sd card; and from the cameras menu, click 'update lens firmware' (read: flash lens firmware). Only takes a minute or so total IIRC; and fixes the 'need to remount lens if focus is extended and pressed in while camera off' problem.
