That's what I figured, haha. I still have no clue how collections work; I should really dig a little deeper into that.
The idea of collections is pretty simple. You can create as many collections and sub-collections as you want, and photos can go into multiple collections, even if you have Lightroom set up to not allow duplicates in the Folders (that's how I'm set up).
So, I have collections that are "categories" representing the various categories of the things I shoot, like Landscapes, Street/Urban Photography, Flower/Plant Photography, Family, Friends, etc. And then there are sub-folders, either with photos from individual shoots or maybe a sub-category if called for.
And, the individual photos are keyworded, so I have different ways of accessing photos.
But that still leaves me with the question why my folders are so weird...!
You know, that's an open question. Somehow, you managed to get two copies of the shoot into different places in Lightroom. To solve that, you'll have to go back and step carefully through all your options/actions.
For one thing, I don't Copy when importing from somewhere on my hard drive, I either Add (to leave things in their current location) or I Move. And, I don't allow duplicates in my Library, at least that's the option I've chosen. I don't know how far Lightroom goes to check/verify a duplicate, but then as a rule my workflow is pretty direct and basic: I use Lightroom to Import from a card/reader into a named folder on my internal "data" drive, and then after short-term work I move (not copy) the folder onto an external "library" drive. And that's it.
So I imagine there is something pretty simple that you just weren't aware of in your process of setting up that folder, I just don't have a clue as to what it was!
