Hey gang. I filled out an online form for sending my lens in for repair (New York) but now Im not sure exactly what to do. HAHAHA I was expecting an email with a packing slip and some instructions or something but I dont believe they send one. Do I send it in its case and box? Just the lens in some bubble wrap? Do they look up the repair by the serial number? Thanks for the help everyone. (I know I could call them but I figured others would eventually have the same question)

Sigma UK ask that you don't send any packaging, just leave the caps on and wrap well in bubble wrap and put in a good strong box, well sealed with packing tape. I sent mine insured.

I only sent lens in bubble wrap with the printed out form from their website. Since mine was covered under warranty they never contacted me but just fixed it and shipped it back. Total turnaround was about two weeks. Mine was 70-200mm needed OS motor replaced.

I didn't want to send in anything that I would be upset if it didn't get returned. I think the did hold onto and reused the same box that I used to ship it back.

^^^^^^^

Just what he said. Print out the online form, fill it out, make a copy to keep for your records, wrap the lens in bubble wrap and send it insured. They will reapir the lens and send it back usually incased in a cheesey cardboard type lens protector that you can reuse the next time you send the lens in. Good luck getting any kind of an email as to the status of your repair or when they ship it back. It is all classified and top secret. It is usually about a two week turnaround so if you have not received your lens back within 3 weeks of you shipping it to them contact them as to the status of your lens repair. They shipped my 50 1.4 to the wrong person, who would not return it back to them, so they had to replace my lens.
