I've been using LR4 for a couple of months and everything has been fine. I've done something to screw up my file importing and can't figure out what.

I have my folders by year and then each month. For example, I have a 2012 folder with a folder for each month in 2012. I'm trying to import some photos from my camera into the July folder of 2012. Instead of putting them in July, it's creating a 2012 folder IN the 2012 July folder and then making a July folder to put them in. I delete the 2nd 2012 folder to import again and it does it again. I can't figure out what I'm doing to screw this up.

Any suggestions? I'll toss in some beer for your help.

Just so you know, the "Subfolder" option is there if you want to create separate subfolder for each shoot in your month. You just go over your naming options. I do that, I create a subfolder for each shoot, give it a date/description name, then copy that over to be the base of my filename (followed by a sequence#).
