You just set up everything the way you want and then save it to a C slot in the menu. You then choose the C setting from the top dial to recall those settings. As Martin has mentioned, these custom profiles save a lot of settings, not just your aperture, ISO, and shutter speed - it saves your AF mode and settings (which is useful if you want to save a particular position of a focus point and/or use orientation-tethered focus points, which it also saves), metering mode, and other preferences. To put the camera in Av mode, you have to move the dial away from the custom setting. The only workaround I can think of is to set up the camera the way you want two ways, one in M and one with Av and save both to a custom setting. Then you can swap between the two custom settings, and all you will be effectively changing is from M to Av and vice versa. At the beginning of time there was absolutely nothing.
