Ha, I think he realised that but wanted something more specific...?

Plastic skull off amazon, lit from one side with a flash and gel, and from the other side with an LED floodlight. Put a smoke machine underneath it.

Made the horns etc with the smudge tool in photoshop, then used a fractalius filter. Blended the filtered image with the original, layered a few of the smoke shots over it and used lots of masks and adjustment layers....

I didn't know he was asking about the more details, i thought he was asking about the effect or the look, but good you replied and answered him which is important.
