I use diy flos that are just 4 T8's per fixture and use 2 fixtures set about 2 ft apart and sometimes put a white bounce on a small stand right below the subject. occasionaly I use a kicker back at the subject from camera left.

My philosophy on headshots is to bring out some personality in the person. For example, the lady up above posing with her hard earned muscles is actually a fitness model and instructor. I wanted just enough of her profile, but also to show her muscles, her "qualifications" as it were. She loves the image and uses it for her headshot - is it a traditional headshot? Not really. But it still works, IMO.
