Address On Forms?

I am in the process of making my business official, and I am wondering about what address to put on several forms. For instance, I need to get EIN, business license (local), state sales tax, etc. On all my marketing information I am using my Google Voice as my phone number and my UPS Mailbox as my mailing address. Since I am a sole proprietor and all taxation is passed on to me, should I just use my home address and cell number on the official business documentation and reserve the fictitious information for marketing and client interaction only? Is there any reason NOT to use personal info on these forms? I am thinking about what gets listed when a business is started, local business licenses directory, etc. I'd rather not have anyone know my personal address and phone number if possible, but it might be unavoidable.

Not sure how this should work and not sure where to look for the answer. Thanks for your help.

You should NEVER use fictitious information on a government form.

Now if you're talking about using a PO box vs a physical address, it's up to you. I've never had a PO Box or other 'mailing address', and never had any problems, but I don't do a ton of business with Joe Public. Most of my business is done with companies who only need my address to mail a check.

Don't think too deeply about it.

If the state paperwork lets you use a po box, then it's totally your choice. If i remember correctly, some require a non po box address. I think a google voice number is fine too. The state doesn't know the difference. I wouldn't consider a google number fictitious either. It's just as real as any other number.

Just know that if google ever changes their service or if you change po boxes, you need to correct the paperwork. Although, same thing could happen if you used your home address, and actual phone number and moved/changed.
