Looking at the hair I'd call added in post. There is a tutorial in the G&N section on how to add these circles but in negative of this with colour on white.

thanks - it all starts with good lighting as without a solid base you would struggle to create the look - this lighting is pretty simple - two sidelights (30x60" gridded softboxes) positioned back facing forward at maybe 30-45 degrees - front light source is a 47" Octobank positioned front and above - I also use a white reflector for fill underneath the front light source. Post process is quite a few steps, too many to detail here, but essentially I use quite a bit of dodging and burning and a plugin (Topaz) to selectively enhance texture via a mask layer.

Bored this evening. Three lights.

Three homemade mini-softboxes at 10, 2, and 7 o'clock.
E-TTL, group ratio of 1:2 (A:B) for the 10 and 2 positions with -1 FEC.
Group C, at 7 o'clock, was also -1. Lots of fiddling with FEC. Would
have been easier with manual but I was curious about how E-TTL
would handle the overall exposure with the black background.

A recent senior portrait I did.

Two lights, main light was a AB800 through a medium softbox camera left. The girl was almost looking right at the light. It was only 6 feet away. The light was at 1/16 power i think... One light behind to capture the light falling snow and rim light her a bit.
