Is it used "under" something else?

Not literally. I don't mean to give frustrating answers. It's hard to give accurate ones that don't mislead.

Is it used indoors or outdoors?

Indoors.

ok - are they always this hard?

Is it part of some type of keychain?

"Hard" depends on the questions. Just guessing is often not the best way to go. A basic question like about the size, would have got to the back scratcher quite quickly.

Would it fit in a shoe box?

No.
In one sense of "use," it is.
It's normally hidden, but "within another" doesn't exactly describe its location.

Hmmm, wear/play ? Is it ornamental?

Yes.

Not ornamental. I don't know what to do with "wear/play."

I agree that just guessing is a poor strategy. The size of the scratcher could be estimated closely enough from knowing that it was hand-held.

I mean, is it something we wear, or something we play with?

I would have had to say it wouldn't necessarily fit in a shoe box, it extends..

Is this essentially a flat thing, apart from the raised part(s)as shown?

It's essentially flat. It has other contoured areas, but nothing sticks out far.

Would this normally be used/found in the bathroom?

The bathroom isn't an impossible place for it, but other locations are more likely.
