Did this for a friend a week or so ago, she needed a head shot for her companies staff page. Spent 15 minutes with her, including setup, shoot and breakdown. The location was 10 yards from her front door, we shot in 3 separate spots and I took about 20 total photos.

24x24" softbox with a 285HV on camera left about 12' away, 1/16th power. It was overcast so the softbox was used to just fill in the very soft shadows in her face and add the tiniest amount of catchlights in her eyes.
Post in LR4 w/ VSCO presets. Skin smoothed in PS w/ OnOne (3 different presets used to smooth and sharpen).
