Strap to fit BOTH a 400 DO IS lens and 5d mkiii - one strap

My sister has a 5d mk iii and a Canon 400mm DO IS lens. She is a bird photographer and usually just carries her camera and lens. She seldom uses a bipod or tripod.

She wants one strap to go around her neck which has the normal two leads to go to the camera body (standard) AND a Y from which a second strap also goes to the strap fittings on the 400 DO lens. So one strap around her neck would balance both the camera body and lens with their own straps. (visualize two standard camera straps meeting into one pad to go around her neck or shoulder). That's basically it but she envisions a single strap coming down and then branching off to the body and lens. Hope that explains it well enough.

Any suggestions?
