As long as the front of the lens is shielded from direct light, and even a lens hood could help with that. However, even the focus ring on the 180mm macro extends only about 5" from the body, the 100mm macro about 4", leaving little space to fit the flash head. Maybe the smallest speedlight could help.

Same type pole, I already owned one, but I slid a 6 inch long section of "pool noodle" on the end and compress it to cause friction on the ceiling. Works well but I've never put anything other than foam core reflectors and flags on it.

You could even get some aluminum foil (or shiny craft paper if you wanted to spring for it. A couple of sheet of foam core and a hot glue gun and build something similar I suppose.
