I work as the primary photographer for a large company, but we're in the process of hiring another. In the past, we've hosted the pictures on a shared drive, but it's a pretty volatile situation - and occasionally people will be unable to access it.

That said, I'm looking for a web-based solution that where I can upload large amounts of pictures, and tag with keywords (preferably on imported metadata from LR) and arrange into albums that other colleagues can access.

We're not wanting it to be public facing at all, so I'd like it to be account access only. We don't need it to be a sharing site, just a database that lives online rather than local drives.
A quick google search gives me a few options like dbgallery, but I was hoping to get some real-world insight, not just whoever maximized their SEO. One option I saw was Widen Media Collective, which looks great, just a bit big/expensive for our needs.

Any opinions?

I haven't used SCC MediaGrid for years and I doubt it is inexpensive, but it sounds like it would meet your needs. Here's the link: http://www.sccmediaserver.com/index.html (external link)

Any reason you can't use flickr and just make the images and albums available to "family"? You can create a list of flickr users allowed to access and no one else will be able to.

FileMaker Pro.

it is extremely robust, yet fairly easy to use, offers instant web publishing, iPad/iPhone apps etc.

there are plenty of pre-built file management solutions for it, or you could build your own. Sounds like you don't have a big list of requirements, building your own might be an option.
