I live and work in "The Triad" region of NC, we have a ton of furniture designers and manufacturers, and subsequently, a ton of large studios, there are ZERO camera stores, much less a place to get things serviced or rent. We are not "small town, USA", as an example of the scope of the photography that goes on here: http://www.kreber.com/#studios
In Charlotte and Raleigh there are a couple of camera stores but both of those towns are 1 - 1.5 hours away.
"most of the world" is not NYC and LA, SF and Chicago. There are many more pros working outside areas that have high levels of support, than there are inside those areas.
not saying that there aren't lots of folks who are tied into certain brands of lighting, but just as people have always been able to jump brands of cameras, they can also jump lighting brands.
or for that matter run multiple brands depending on what they need for that particular shoot.
