Regarding Hurley's "forehead forward" suggestion, here is a comparison. The first is based on the Hurley suggestion of bringing the forehead forward, and the second her without it. These were taken back-to-back. Taste is subjective, but I think the first is more flattering. (Ignore the weird cropping. I just took a screenshot of them together.)

I think your understanding of Hurley's technique is off, at least if you're referring to what he demonstrates in "It's all about the jaw." It's not about tipping the forehead towards the lens, it's about bringing the entire face towards the lens. Imaging a vertical plane between the model's forehead and chin, and then clamp that so the angle can't change and drag it closer to the lens.

The left image the model's head is tipped slightly down. On the right her head is tipped slightly up. This is a good example of how an image changes based on the angle of the head. On the left the eyes are more prominent and the nose and chin seem longer and thinner, and the subject looks more "approachable.' On the right the chin face seems shorter, chin stronger and she looks a bit standoffish. The angles also affect the model's neck, looking shorter on the left and longer on the right. It's hard to say which of these I like better; they are both wonderful shots (except I'd try to get a little more expression). But I think I like the left more than the right capture.

I like the lighting on #1 but, in the future, you should turn the body away from the lens to narrow the shoulders; even more so with larger models.
I don't think the lighting is flattering in the second. I think it would have been better if the light was brought around front, closer to the camera. I also would have worked with her to tone down her smile.

Nice shot, but the eyes seem a little dark to me. Perhaps moving the key light back or lower?
I assume you were going for the serious brooding look? I'd normally try to get a more pleasant facial expression.

Nice lighting and composition, although I think it's cropped too tight. I don't think the straight-on body angle was a good choice here, but cropped as it is it's hard to tell. But way too much processing, she looks like a plastic barbie doll.
