For office work (Excel and the like), I use a Logitech Trackman Wheel - it's a trackball for the thumb.

For photo editing, I use an old Wacom Bamboo, coupled with the touchpad on the Macbook Pro.

Intuos 3 tablet + stylus, 6x11. Haven't used a mouse in years. Even with marathon editing sessions of 8 hours or more, no pain. Use it literally for everything, editing, surfing, whatever.

Can't imagine anybody using anything else...
