Pre-paid envelope printing

Some time ago, someone posted a layout of a sample of their ball team pre-paid package pricing (pdf version) on printing those on envelopes ourselves. I need to do that this year, print my own pre-paid envelopes to hand out to parents for ordering packages/prints ahead of time. I cannot find that now, does someone else have that or something like it or know where I can go to download it? It originally was for 4 1/8 x 9 1/2 size envelopes. Maybe there is a template out there for different sizes that could be customized?

Check with your lab. Many have extensive resources like that.

I did that several years ago for various school related jobs. I used Microsoft Word to make the order form for an envelope. It took a little bit of time though it wasn't too aggravating, say and hour or so. I had to print a couple or three test envelopes to make sure everything looked ok. The only problem I had was tending my printer so it wouldn't eat envelopes.

Not my lab. Besides, labs charge too much. Far cheaper to print your own on your own printer. Someone last year posted a template or a layout they used and said we could use it also but I lost it and have no idea who it is now.
