5D Mark II, 100mm 2.8, Kenko Extension tube set, 580ex, $25 Flash shoe cord, $20 Flash Bracket that can be switched from Portrait to Landscape orientation or to change position of flash, and finally a $5 Pan and Tilt Platform Mount (the small ball head connecting the flash to the bracket).

I also have a ring tripod mount on the lens which I can place another flash if necessary, but it gets too top heavy. I'll end up using it to mount a piece of white paper as a reflector.

Just wondering that myself. Can you post the link to where I can purchase the ballhead and bracket?

I think this bracket is a Stroboframe Stroboflip VH 2000

It's similar to the stroboframe, Keesbeest mentioned, but mine is not $100 :) It's a Hong Kong knock off version that only cost about $20 :D

I got the Bracket, mini ball head, flash cord, and tripod mount all from the same website. Overall it cost me less than $75 for the Macro Rig.

Thanks for the link, Supastudio. One more question, though: does the bracket itself have a threaded hole for tripod mounting on the bottom? I like the looks of the bracket, but would probably want to be able to mount the entire rig on top of a monopod...

Looking at the product photos, it appears that it would be possible...

Yes it does have a thread hole for a tripod. Its also located more forward than the camera body making it better balanced on a monopod or tripod. One thing about the bracket: it gets to be a little shaky when placing so much weight on it. It gets worse when tilting the flash forward and above, but I'm not worried at all about having it fall. It's just a little scary when it bounces around a lot :)

Yeah, there is definitely plenty of crap you need to go through to find the decent products. They recently acquired ring flashes similar to the orbis but only 30 bucks :D
