I just checked my camera and it was only back button focus and shutter didn't do anything.

The EXIF data was ISO 400, f/2.2 and 1/200 sec

go outside and shoot something in the grass that is a decent sized object so the focus sensor cant miss in single shot. take multiple shots refocus each time and see where the focus plane lies in the grass. if its consistent with being infront then most likely is a lens issue that MA would fix but your body doesnt support that.
from post you put up I know the sensor area is larger then the focus point you see but there is no way it is that much bigger to miss by that much and grab something that much further from her face.

what focusing mode were you in? i assume you took a lot more than just one photo...were they all out of focus?

A lot of people who either don't understand back button focus or don't read well, dog piling on the OP here. Looking at the image he's got about 1.5 ft DOF and would need a ~15 MFA adjustment. OP it may be cheaper to hate your 85 than your rebel, though a 6D could "fix" your 85 and give you better IQ.

The easy question, which I didn't see asked?, is whether this is the full frame or is this a crop? (ie. was the shot posted just a resize of the entire frame, or was it a resized crop of the entire frame?)

Not sure if you are being pedantic, or just trying to lighten up the thread? I will assume the latter. :)

Based on comments when the 6D came out, I think people believed the 6D was a full frame rebel. :D The 6D might be the answer here as mentioned earlier.

I cropped in just a little bit because I had too much blurr in the foreground. I will do the object on the grass test tomorrow to see if it's the lens. I did have another image from this series that had her face in focus more. So maybe the lens or camera didn't lock in focus. I have another session tonight and is there anything I can do? Maybe shoot with an high aperture just to make sure I can nail focus better?

Could we please see the full frame? It could help validate many of the comments here around the AF points vs the actual AF sensor vs the contrast throwing off focus.

I would do some tests before doing a session. Shoot wide open when you do tests.

full frame wont solve your problem, you need better AF or live view. Your AF points are not trustworthy. Hate to even mention it, but sony has "Eye focus" and you can eliminate the guess work for this sort of thing. It puts focus point on the eyeball... I routinely deal with much thinner DOF and have no issues with AF or live view.
