All the time, When I'm traveling I do prefer to put a body in a pack without lenses, and after that on area pick lenses to mount on it for a specific shoot.

Honestly, it never occurred to me to put my camera in the bag WITHOUT a lens attached. Not having a lens mounted would allow me to pack more in the bag. Not sure that matters much. I usually only leave home with 2, and very rarely 3, lenses. I don't like carrying a ton of stuff I'm not going to use.

Same with me. I have a Think Tank Retro 30 and usually have a 5Diii with a 24-105 attached in the "middle" compartment, a 70-200 f/2.8 on one side and a 17-40 on the other. A bit heavy at times but I've got my bases pretty well covered with that combo.

I keep a lens mounted. One reason I try to minimize lens removal just to reduce the chance of getting something into the camera. The camera is also ready for use too, as mentioned earlier.
