Reason for selling: I'm mid-switch to Sony and I picked up the Sony/Zeiss 55mm f/1.8 so I'm covered on the new system at 50-ish mm.
I purchased this new from Adorama on 3/16/15 (yes, you can have the receipt with purchase - in fact it's already packed up inside the box). It has been a flawlessly performing lens for me and is insanely sharp. I've absolutely loved having it and after my negative experience with the 35mm Art, it has restored my faith in Sigma's Art line.
It's in great condition (no marks on the glass and the body looks great, only the lens hood shows any marks) and I've never had a single issue with it. I've shot weddings, events, maternity, family, candids, and practically anything else you can imagine with it. It's AWESOME. It works great with Servo AF, Lock on AF, and also with live view AF as well.
I have fine-tuned the focus of this lens with the Sigma Dock and I'm leaving the settings as-is for the buyer to evaluate as I've had it on a 6D, 5D Mark III, 5D Mark IV, and 5Ds and it's been great on all of them. If the current settings work for you, great! Work done! If not, you could certainly tune them yourself.
Not pictured: the cord and documents for the Sigma Dock and the documents and the lens case for the lens. EVERYTHING that came with the lens and dock will be included for you.
After you've used this lens for a day and you've put your own fingerprints on it, it'll be practically indistinguishable from new.
Price ($685) includes PayPal fees and insured Priority Shipping via USPS so there's nothing for you to worry about.

EDIT: A full-size image shot at f/1.4 (ISO 100 but raised in post .55 stops). I chose this image as it illustrates foreground (a little) and background blur with foliage - which often demonstrates a worst-case scenario. At f/1.4, we're counting eyelashes and beard hairs, folks! This lens is incredible. https://farm5.staticflickr.com …93608941_13c64b418a_o.jpg
