It depends on what I'm going for. No set rules but the ones the client requires.

"The correct settings to achieve my vision for the image" is the only worthwhile answer anyone can give. There are countless different types of portrait and they each need different settings. Without knowing which of these many types you want how can anyone talk about specific settings?

Do you have a particular style you are trying to achieve?

f1.4, ISO 100, 50mm, 1/250, AF spot focus

then also

f5.6, ISO 400, 30mm, 1/250, AF all points

then also

f3.2, ISO 200, 70mm, 1/200, single point AF

F/5.6, ISO100, 1/160, single point AF, custom white balance.

(using flash in studio or in a dimly lit room)

I select focal-ratio based on depth of field that I want (which varies with single vs groups).
My shutter and ISO will be based on whether I'm in studio with lights, outdoor with or without lights, and whether or not I need a faster shutter for motion freezing while blending ambient & flash.

There's no one set of values.

For individual or couples portraiture, I select FL in studio based upon the typical 8-10' shooting distance, to frame based upon the type of portrait
(full length standing vs. 3/4 length vs. waist-up vs. head & shoulders vs. tight headshot)

I tend to select f/stop based upon achieving some separation of the subject from the background via DOF control (but never ever seeking the razor thin DOF so commonly sought by hobbyists.)

Shutter speed and ISO are based upon the lighting intensity (available light vs. all studio lighing)
