I remember somewhere there was a site where you would fill in the blank fields and it would give you an estimate to use your image in a book. Anyone remember the site?

A lot depends on the press run, uniqueness of your image and regions that the book will be sold. If it's less than 40K press run I can see an image being worth $400-$1200 with the cover bringing the higher end. I would use the calculators on websites like Getty Images to gauge your response.
